This form must be submitted to initiate the funding process.
Loan Requirements
- You may borrow up to $10,000 per fiscal year to a cumulative total of $30,000 over five years.
- You may borrow any amount (rounded to the nearest $100) up to $10,000 in a single year. It is recommended to request the full $10,000.
- Once your loan request has been processed, the amount cannot be increased; however, you need not borrow the entire amount initially requested.
- Funds will be disbursed according to your academic calendar:
- 2 installments for semester calendar
- 3 installments for quarter calendar
It is your responsibility to submit the
Student Enrollment Verification Form (SEVF) (.pdf) form each semester or quarter for subsequent disbursements.
The deadline for submitting the Loan Request Form is August 18, 2018.
